Spotlight: Home Health Services: Identifying “at risk” family, friend or neighbors.
Do you know someone who lives alone, is elderly and frail? Someone who has had frequent falls and keeps calling the fire department to come pick them up? What about someone who struggles with preparing meals and managing their medications?
These are all signs that home health services may be indicated. First, in order to receive home health services, a person must be home bound. This does not mean they never leave home. This means they can not leave their home without help (cane, walker, wheelchair etc.), or they are generally unable to leave home without a major effort.
Ask yourself, does your friend/neighbor:
• Have difficulty understanding what his/her disease(s) process is and how to manage it?
• Make frequent visits or phone calls to the doctor’s office?
• Have a history of falls, balance, or other safety issues?
• Live alone, with or without a close-by relative?
• Have multiple medications to manage?
• Requires frequent emergency care or have recent/multiple hospitalizations?
• Have shortness of breath, even while sitting?
• Have an unusual weight change lately; gain or loss?
In general Medicare pays for Home Health Services. Services may include Nursing, Physical Therapy, Occupational Therapy, Speech Therapy. If you know someone who may benefit from Home Health, please contact their Primary Care Physician and request a consult (order) for Summit Home Health & Hospice for an assessment to identify the best way to meet their needs.
